The discount% is applicable on Price List Rate of an item. If Item's Price List rate is blank, entering discount will not have any effect on the item's Basic Rate.
To pull Item's Price List Rate, you should:
Create Item Price master from:
Item >> Add/Edit Item Price >> Add New
Or
Make Price List Rate field in sales transactions editable from:
Selling >> Selling Setting >> Allow editing Price List Rate.

Is there a reason for this approach? It seems a bit confusing especially for Sales Users trying to apply a discount at the point of sale (in a scenario where Price List has not been setup).
Is there anything wrong with being able to apply a discount to the Basic Rate?

This feature is available in the current version. In the transaction, below tax table, in the additional discount section, you can apply discount over Net Total and Grand Total. This Discount Amount will be effective over Basic Rate of an item.
@umair What I would like to do is give item discount in the “sales order item”, because some items in the same order are not discounted. currently it is hard coded for the price list rate, is there a way to have it discount on the new rate that is input? Thank you for your kindness.
In the Sales Order Item table, discount can be applied on the Item’s Price List Rate only. If entering item rate manually, you should enter rate in the Price List Rate field only. You can make this field editable from:
Selling >> Setup >> Selling Settings
Check field “Allow user to edit Price List Rate in transactions”.
