Custom fields for linked Contacts, such as on Lead DocType

Hi, how are these fields implemented on Lead DocType? I wanted to re-do them for Competitor DocType, but it does not seem to be possible:

You can always with Client Script implement more feature on refresh() event on Competitor DocType
