How can I customize this form?
I customized the ticket doctype but that changes are not reflected in the customer portal’s new ticket form.

You cannot customize this page as of now.
When will this page be customizable? Having custom forms for different Ticket Types sounds like an important feature to have on an Helpdesk.
For now, you can add additional fields. First add the custom fields in the HD Ticket doctype and then go to HD Ticket Template to add custom fields.
