Doctype Module Error

I am trying to create a new doctype under a module.

But after entering the fields and saving it is showing an error “Mandatory fields required in DocType
Module”
I beleive i have entered all the fields.

What is the issue ?

The error i am getting while saving doctype

I am choosing Module as College Library from dropdown but still it is throwing the error !

same is case for Permisssion Rules.
