Error In Customize Form

When add new custom field in customize the form and save after the error msg will be displayed anyone has a solution for this…Please Help.

As per the popup instruction, kindly refresh the form and try.

Thanks, @Manjunath_Sajjan for the reply but I tried everything it didn’t work…!! I refresh the form, give a unique name, clear bench cache still got an error…!!
