You can make a Web Form linked to a Feedback doc type (which essentially has a linked field to the project) …
The web form will be linked to your ERPNext Web Page (Portal). Customer might not require to login, or in some case if you want an authenticated response, you can create a customer user account and empower user to login and then fill the details.
