How does Self appraisal work?

How does Self Appraisal on ERPNext work?

I’m trying to achieve this:
1: HR manager to send out appraisal forms to employees
2: Employees appraises themselves and submits appraisal.
3: Manager receives appraisal and can approve, deny or edit.

I’ll appreciate a timely response.

You can create an appraisal and apply workflow on it for approvals.

Am not sure it can work that way.

The employee will need to score himself on one column, and the supervisor scores him on another column.

As it is ERPNext has only one column for scores
