Use the customize form tool, accessible from the doctype list page, and then there should be a button in the upper right corner allowing you customize the child table for items.

We define custom field in item Form CA No and link to Purchase Receipt Form when we select item code CAT No automatically link to item form and print in Purchase Receipt.
