How to upgrade my custom app

Hi,
If I have custom app I build it with frappe v14, erpnext v14.
but now I need to upgrade this custom app to working with frappe 15, erpnext 15

How I can do this?

Please attempt migrating your custom application from version 14 to version 15. If the migration is successful, your custom application should function properly. However, if you encounter any issues in version 15, adjust and update your application’s structure accordingly.
