are you logged in as “Administrator” ?

exactly I logged in as administrator no other account are there only one account there

You can open new form from Awesome Bar as well.

I got the solution:
actually if we use administrator username and password admin root password at new-app generation we can get “Doctype” other wise we can’t get it
