Item name not displayed when printing purchase order

Hi,
I am using the latest version of ERPNext. When i set to print purchase order, the item name cannot be displayed. I tried to customize the form too but again facing same issue, can anyone assist me how to do it

Can you see the above image description column, item name cannot be displayed. After i tried print format builder but it also doesn’t work and i attached below for your reference
