No fields showing in "Add Empoyee" page

Hello.
I am having a problem adding an employee in the HR module. When I click “New” I get the add page but no fields are visible. I am using an Admin account so permissions shouldn’t be a issue.

Please help. I will include a screen shot.

Can you checked browser’s console log? Any error did you get?

Did you written any custom script? Which branch you are using? Version of ERPNext?
