Not Displaying ERPNext Letterhead Created in Custom Print Format for Salary Slip

When I select the standard salary slip format, the letterhead appears correctly.

When I use the custom HTML format, the letterhead and footer do not appear.

Could you advise to make the ERPNext letterhead appear with custom HTML formats ?
