Purchase Order Item Showing Received, Did not receive

I just submitted a purchase order and one of the items is showing a green circle next to it. When I open the item it shows that it has been received. How is this possible? I did not receive the items, nor create a purchase receipt. How do I stop this from happening?

What is the quantity of the item received?

Can you provide screen shares?

The item is showing that the quantity received is the same as the quantity ordered. I did not receive this items in the system…
