PWA Notifications Created Without Push Relay Enabled, But Not Showing in Bell Icon — Why?

In ERPNext v15 PWA, I noticed:

  1. PWA Notifications are created even though “Enable Push Notification Relay” is off. How are these PWA notifications generated without it?
  2. And if it does not depends on that option then why these are not showing up in the bell icon dropdown. Manually created notifications do appear.

What causes this? How can I fix or debug the missing notifications in the bell icon?
