Unable to create Quality Inspection from Stock Entry

Unable to create a Quality Inspection from Stock Entry (Manufacture Type/ Backflush FG)
On Clicking “Create new Quality Inspection”, new Quality Inspection is not created as intended and the browser console shows following error (Refer attached screenshot)

Can you upgrade your site? This seems to be working fine in v12.0.8
