What is the correct way to Modify Module Navigation?

It looks like the way modification of the Module Navigation system works changed from version 12 to version 13. I’m not seeing where there is documentation explaining how to do it in version 13.

How do you?
a) Add new “Module” links to the “Module” menu?
b) Modify the “cards” and “shortcuts” shown on a standard Module page?
