Who did create the data in sales invoice

i entered with admin user and add one item to sales invoice
after refresh the page there are two items owner by deferent user with different data
??!

i could not solve the data till now

the user create the doc before 2 days with one item now i saw new item
i removed add button form the screen

this code effect ?
